Carvex needs repair (UK)

At work today my carvex decided to play up.  It was becoming temperamental and going on and off when I was using it, then decided to stop altogether.  Its got over another year on its warranty what do I need to do next, take it to the dealer and they deal with it?

This will take you to the online repair form. Fill out the relevant details and send back. This will start the collection process for the repair to the machine.

I have had to send mine ( 400 ) back twice, once because the switch wouldn't work and secondly I couldn't get the blade to lock properly . both times I couldn't fault the service.
